Job description
We are currently working in partnership with a Charity based in Bristol, who are recruiting for an Accommodation Pathways Lead which is full time and can start within the next few weeks. The role will hopefully be a temporary to permanent one.
The role will be primarily based in Bristol but there will be some travel required to Somerset and Wiltshire. The rate of pay is negotiable on experience and there could potentially be one day from home.
The ideal candidate will have a car and an Enhanced DBS and have a strong background working with Housing Pathways.
Duties will include (but are not limited to):
- Working in a multi-disciplinary team that focuses on supporting people in secure mental health hospitals as they transition to community living
- Managing a member of staff and help support and guide them throughout their day to day
- Spot gaps in provision, supporting, challenging and navigating system complexity, coordinating accommodation plans, liaising with housing providers, and working closely with clinical staff to ensure smooth transitions for clients from secure settings to community living
- Contributing to shaping the housing strategy, monitoring the effectiveness of accommodation pathways, and providing guidance to team members on best practice for complex cases
- Ensuring appropriate access and support to housing by drawing on existing resources and expertise at the organisation and in partnership with others
- Developing a range of community-based accommodation options for those with forensic needs
Experience required:
- Proven knowledge of support needs of people with mental health difficulties
- Knowledge of existing housing pathways or supported housing models and how to utilise these to meet the needs of people with forensic needs
Skills, knowledge and expertise required:
- Proven experience working in a community-based service (e.g. supported housing service, community mental health team, day centre, education)
- Proven experience of effective risk management of community based services and working within standard operating procedures
- Demonstrable knowledge of systems change and ability to embed a systems thinking approach.
Working hours:
- 37.5 hours per week
- Monday - Friday, 9am-5pm
- Potentially some home working
