We are currently working in partnership with a Charity/Not-for-Profit based in Monmouthshire, who are recruiting for an Outreach Worker on a permanent contract. The position is due to start immediately, however the client is happy to wait for the right person to finish their notice. The salary is £23,175 - £25,235 per annum dependent on experience.

The ideal candidate will have experience lone working, working as a team and have worked with a similar client group. They will have access to their own vehicle and be willing to travel to Monmouthshire, Chepstow and Abergavenny.

Duties will include (but are not limited to):

• Providing general support to 25 residents, who might have mobility issues and completing home visits when necessary

• Supporting individuals with drug and alcohol problems who have been given temporary accommodation

• Working with clients to understand their individual needs and create tailored support plans

• Providing continual advice and support about their drug/alcohol issues

• Signposting to appropriate services when required, e.g. Social Services, the Police, Hospitals

• Enabling clients to maintain accommodation and develop independent living skills

• Liaising with external agencies including the Local Authority when required

• Completing and maintaining up to date paperwork, including risk assessments, and files for each client

• 35 hours a week – Monday to Friday
